List of Annual Library Forms and Deadlines by Month

Technology Plan Comprehensive Technology Plan must be submitted every 3 years for eligibility for E-rate discounts if filing for Priority 2 services and equipment. State Library will send certification form showing approval of new plans only. An update form required annually if filing for Priority 2.	Technology Plan Update Form	April 30, 2012
Filtering Certification Required annually for compliance with RSMo 182.825 and 182.827.	Certification of Compliance Form	March 31, 2012
State Aid Certification Required annually to receive state aid funds approved by the Missouri General Assembly. Please note -- Each type of library district has a different form.	State Aid Certification Forms Forms must be completed, signed, notarized, and mailed.	July 31, 2012
List of Trustees Required to verify information on state aid forms and update our mailing list	View forms here or request by mail.	July 31, 2012 & as board changes
Statistical Report Required annually. Beginning in 2008, public library statistics are submitted to the Institute of Museum and Library Services (IMLS) to be aggregated at the state and national levels.	Libraries will receive instructions and login and password information in October. Libraries must submit their data through the online Bibliostat Collect program. Only libraries without Internet access may submit a paper form to the State Library.	December 31, 2012
Summer Reading Program Survey Collected annually, to continue receiving Summer Reading Program materials from the state library.		September 7, 2012
